We conduct physicals for:

• Athletic participation (middle and high school athletics, pop warner, cheer, etc.)
• Boy and Girl Scouts
• Pre-employment
• Annual check-ups
• Department of Motor Vehicles (DMV) and Department of Transportation (DOT)

It would be best to bring in your form to our office, but call to check if we have your form in our database. Groups of 50 or more may be eligible for our sports physicals fundraising program where we can come to your site.
